Admission

1. Documents required on the day of admission

Please complete admission procedures at the Admissions & Discharges Reception Desk ( located on the 1st floor of the inpatient ward, beside the main entrance ).

( 1 ) Application for admission
( 2 ) Agreement regarding hospital room charges ( as needed )
( 3 ) Patient ID card
( 4 ) Japanese national health insurance card, or health insurance papers
( 5 ) Written consent to wear hospital garments ( a )

( a ) Hospital garments cost 70 yen per day. If you do not wish to rent hospital clothing, you need not submit No.5 above.

2. Please report to the nurses station of the appropriate department by 10:00 a.m. on the first day of hospitalization. A nurse will show you to your room.
