1.事前の準備-1

奥様の婦人科検診を事前に済ませ、結果を紹介状として準備いただくと、話が多少早く進む場合があります。済ませておかなくてもかまいません。検査項目は必須ではありません。検査していても、慶応義塾大学病院で再検査する項目もありますし、あくまでも余裕があればとお考えください。

1. PREPARATION BEFORE VISITING THE KEIO UNIVERSITY HOSPITAL-1:

Please advsie your wife to complete her gynecologic visit in advance and to request a letter of introduction with detailed test results. This may speed up the process. Please note that this is not a vital requirement. Keio University Hospital may do the same test if necessary. Please deem the advanced preparation as an option.

感染症：

HIV抗体、HBs抗原、STS、TPHA、HCV抗体、クラミジアIgG. IgA

基礎ホルモン：

月経3～5日目にTSH、LH、FSH、プロラクチン、フリ－T4、抗核抗体

HIV antibody:

HBsAg STS TPHA HCV antibody chlamydia IgG, IgA

hormonal milieu:

(3-5th day of menstrual cycle) TSH LH FSH Prolactin Free T4 antinnuclear antibody (ANA)

その他、奥様の不妊傾向について、確認をしていただき、結果を紹介状にしてもらっておくのも有効です。繰り返しますが、諸事情から地元での検査が困難なことも多く、これらは慶応でもやれますので、無理はしないでください。

OTHER NOTES:

It might be helpful for the Keio University Hospital if the letter of introduction prepared at the local hospital contains detailed description of your wife's status of agenesis. Please also deem it as an option as the local hospital may not have capability to do this test, and Keio University Hospital has capability to do so.

2.事前の準備-2

荻窪病院の花房医師からの紹介状を入手してください。荻窪病院受診時に受領していなかった場合は後刻の郵送もいたしますので、ご連絡ください

2. PREPARATION BEFORE VISITING THE KEIO UNIVERSITY HOSPITAL-2:

Please obtain a letter of introduction from Dr. Hanabusa at the Ogikubo Hospital. The letter of introduction will be mailed to you in case the letter preparation was not completed on your visit.

3.事前の準備-3

荻窪病院を通して慶応義塾大学病院の初診日をとってもらう。

原則火曜日か木曜日の産婦人科久慈先生の初診外来への予約になります。なお、事情のある方は一応、小島カウンセラーに相談ください。可能かどうかは分かりませんが、調整を試みます。

3. PREPARATION BEFORE VISITING THE KEIO UNIVERSITY HOSPITAL-3:

Make an appointment at the Keio University Hostpital THROUGH the Ogikubo Hospital.

The appointment day will be either Tuesday or Thursday when Dr. Kuji of the Department of Obstetrics and Gynecology is present to meet the first time visitors. If you require special arrangement, please consult with Dr. Kojima as he might be able to shift the reservation days.

4.久慈（くじ）先生の産婦人科外来初診の日

時間的には午前の初診受付から説明終了まで、少なくとも3～4時間（終りが午後4時になった例もあります）を考えてください。改善努力はしていますが、大学病院はなかなかかかるようです。受付は遅くとも11時までには済ませてください。

紹介状、基礎体温表と健康保険証を持参してください。

注意.男性の精液採取と検査は原則火曜日に実施されます（火曜日でも検査できない日がありますので、予約時に確認してください）ので、木曜日の診察の場合は、ご主人様は再度、慶応義塾大学病院に行っていただくことがあります。
初診以降の予定については初診時に久慈先生と相談してください。

4. WHEN YOU SEE DR. KUJI OF DEPARTMENT OF OBSTETRICS AND GYNECOLOGY FOR THE FIRST TIME:

a) Required time:

First visitor registration in the morning till the end of the meeting with Dr. Kuji will take at least 3-4 hours (sometimes till 4 pm). The long waiting time is a typical nature of Japanese university hospitals. Please make sure to complete the first time registration before 11 am.

b) Items to bring:

Letter of introduction, primary temperature record, health insurance card.

REMARK FOR MEN: Sperm collection and check day is done only on Tuesday (sometimes even Tuesday is not available for this procedure, so please double check when you make appointment with the Keio University Hospital. If your first visit to the Keio was on Thursday, husband is required to re-visit the Keio on another Tuesday.

c) Schedule after the first visit:

Please consult with Dr. Kuji for the further schedule.

5.費用等

当日、予想される検査と費用ですが、事前検査の結果がなく、最低限の検査（女性側のHIV検査と男性側の精子洗浄検査）を行った場合、2万円程度です。但し、治療をはじめる際に特殊洗浄の実費として体外受精の際に（寄付名目で）10万円必要となります（これは体外受精費用には含まれません）。なお、検診の結果、別な検査が急遽必要となる場合もありますので、保険証の準備と若干の余裕をみてください。

当日、予想される検査と費用ですが、事前検査の結果がなく、最低限の検査（女性側のHIV検査と男性側の精子洗浄検査）を行った場合、2万円程度です。洗浄に関しては研究費で無料となりますが、洗浄後精子の保存費として10万円程度がかかりますが、体外受精のコストの一部として後刻の請求となります。なお、検診の結果、別な検査が急遽必要となる場合もありますので、保険証の準備と若干の余裕をみてください。

5. COST ON YOUR FIRST VISIT:

If you do not have advanced test results, the minimum test (women's HIV test and men's sperm cleansing test) will cost around JPY20,000. Please be advised that you are required to donate JPY100,000 to initiate IVF treatment.

6.参考事項-1

実施日前は二週間、慶応義塾大学病院に通院していただくことになります。

6. REFERENCE 1:

You will be required to visit the Keio Hospital for two weeks in prior to the IVF treatment.

7.参考事項-2

体外受精の場合、地元医療機関との連携がとれれば、慶応義塾大学病院への通院は一週間に短縮され、その後、実施となります。なお実施後には最低二泊三日程度の入院を考えてください。

7. REFERENCE 2:

For IVF, if your local hopital has capability to do joint operation with the Keio Hospital, you will be required to visit the Keio Hopital for only one week before the IVF treatment. Please assume that you will be staying in the hospital for at least two nights after the IVF treatment.

8.参考事項-3

海外で生活している方、初診時に婚約中の方などには、実施時までに結婚証明書の持参をお願いする場合があります。

Certificate of marriage might be requested for IVF treatment if you are living in overseas or if your couple is in the state of engagement (not officially married yet).
