Reception procedures for outpatients 
( 1 ) Registration of first-time patients 
- Please come to the Outpatient Reception Area on the first floor of Outpatient ward during 8:30 a.m. - 11:00 a.m.
- Please fill out an "Application for Consultation" form ( provided at the hospital ) and present it at Reception counter No.3, together with your health insurance card and letter of introduction ( if any ).
- Outpatients without a letter of introduction may be required to pay a special healthcare charge of 3,150 yen ( tax included ).

If you continue to receive consultation without presenting a health insurance card when requested, you may be charged the entire amount of your medical costs.
( 2 ) Reception of Re-visiting patients with appointments 
- Please proceed to "Reception Machine" on the first floor of Outpatient ward to obtain a check-in slip for your appointment, then proceed directly to the designated department and wait until the number on your check-in slip is called.
- Please submit your health insurance card at Outpatient cashier No.1 after the first consultation of every calendar month.
( 3 ) Patient ID Cards 
Your Patient ID card can be retained for use in all departments of the hospital. Please take good care of it and be sure to have it with you whenever you visit for consultation.
( 4 ) Payment 
- On completion of your consultation and examination, you will receive a plastic folder containing your relevant documents. Please submit this folder, along with your health insurance card, at Outpatient cashier No.1.
- You will receive a numbered ticket at Outpatient cashier No.1.
- When your number is displayed on the electronic board, please settle your account at the automatic accounting machine or at Accounts Window No.7 or No.8.
- Patients with prescriptions to be filled at the hospital pharmacy will receive a receipt with a reservation ticket and a claim ticket for their medicine.
- The following methods of payment are accepted: Cash, credit cards ( VISA, MASTER, NICOS, JCB, AMERICAN EXPRESS ) and debit cards.
- Please have your appointment card at hand as it will be required when paying.
( 5 ) Prescriptions 
(a) Prescriptions for External Pharmacies ![]()
Please obtain the appropriate prescription at each department.
( b ) Prescriptions for the Hospital Pharmacy ![]()
- You will receive a claim ticket for the medicine, when paying. When your number is shown on the electronic display board, please submit your claim ticket at Hospital pharmacy No.12 and collect your medicine.
- If you have any questions about your medicine, please do not hesitate to inquire at Window No.11.
- Also, as part of the improvement in hospital services, and with the co-operation of the Niigata City Pharmacists Association, we are now able to offer patients the option of having their prescriptions faxed directly to external pharmacies. Please feel free to make use of this time-saving service.
